If You Feel You Need A Home Visit

All home visits are clinically assessed and will be granted where a patients medical condition means the cannot reasonably attend the surgery. Each request will be assessed by an appropriate member of the clinical team, who will decide the safest and most appropriate way for you to receive care. You do not need to fill in an online form.  

Where home visits are appropriate and worthwhile

• If you or the patient is terminally ill and cannot reasonably attend the surgery
• If you or the patient is bedbound because of their medical condition
• If you or the patient being moved would cause harm or significant deterioration
• If it is not medically appropriate for you to visit the surgery  

Where home visits are not appropriate

If you are medically able to attend the surgery or primarily due to the following reasons:

• Not having access to a car or other transport
• Difficulty arranging a lift or accessing public transport
• Childcare or other caring arrangements
• Work or other commitments
• Poor weather
• Personal preference or convenience
• Being under the influence of drugs or alcohol
• Emergency medical situations
